Schedule Your 1st Appointment
Call or text our office at 615-550-1800 to schedule your appt. We do not require a referral, but your insurance carrier may.

Prepare for your visit
Complete prior to your appointment:
New Patient Registration Packet -These forms are sent automatically via text and email once you secure your appointment with us.
Insurance card(s)
Pertinent medical records – If records need to be sent to us, we will need you to complete a medical release form.
List of your current medications (or the bottles)
Any imaging discs relevant to your visit (your referring doctors may send the report, but YOU need to bring the images).

Appointment Day!
Arrive on time for your appointment. No need to come early. This applies ONLY if you have completed Step 2.
Check-in at the front desk Bring your up-to-date insurance card(s) Pay your copay (if you have one)

After your appointment
If you need to talk to us between appointments, please text us at 615.550.1800. You can also call – just be aware we take over 200 calls and texts a day so if we cannot answer immediately, you will be placed in a call queue. If you do not want to wait in the queue, please leave a message. If you don’t leave a message, we have no idea you have called. This happens a lot (messages not being left) so again, please leave a message if you want us to call you back.

How can I inquire about my bill or payment options?
You can reach out to our billing department at (615) 224-8394 for inquiries about your bill or to discuss payment options and plans.
What is the billing process for consultations and procedures?
After your neurology consultation or procedure, we will submit the bill to your insurance provider. You may receive an Explanation of Benefits (EOB), followed by a statement from us for any remaining balance.
Can I set up a payment plan for my medical bills?
Yes, we offer flexible payment plans to help you manage your medical bills. Contact our billing department to discuss and arrange a suitable payment plan, but please keep in mind that we may not be able to make future appointments until your payment in full has been received for services previously rendered.
What payment methods are accepted for medical bills?
We accept most payment methods, including credit/debit cards, cash, checks, and electronic transfers. You can also make payments online through our secure portal.
We do not accept deferred payments such as those from Workman’s Compensation. We also do not accept Care Credit.
What payment methods are accepted for medical bills? Are there any financial assistance programs available for patients with financial hardships?
We understand that medical expenses can be challenging. While we do not have financial assistance programs for any of our services, there are many financial assistance programs for medications. Please discuss this with your provider or pharmacist.
